Lower extremity imaging is essential for diagnosis and management of many leg, hip, knee, ankle, and foot conditions. Imaging tests such as X-ray, CT, MRI, and Ultrasound play a key role in the diagnosis of fractures, joint disorders, soft tissue trauma, and vascular diseases. Fractures of the lower extremities are one of the most common fractures, particularly in older adults. About 2 million osteoporotic fractures occur annually in the United States every year, with a significant proportion being lower extremity fractures, most commonly the hip and ankle (1).

Radiologic technologists need to have a definite knowledge of lower extremity anatomy to position properly, optimize image quality, and interpret results appropriately. Imaging technologists also need to take into consideration a variety of pathologies, ranging from osteoarthritis and fractures to soft tissue injuries, which necessitate different imaging techniques.

Each of these modalities plays a crucial role in the diagnosis of various pathologies. X-ray remains the gold standard for the assessment of fractures, dislocations, and joint alignment, while CT scanning provides high-definition 3D imaging of complex fractures and soft tissue pathologies. MRI with its superior soft tissue contrast plays a crucial role in the assessment of ligament, tendon, and cartilage injuries. Ultrasound is a valuable tool for the assessment of vascular pathologies such as deep vein thrombosis and interventional guidance for soft tissue pathologies.
